Would the creation date of a domain ever get reset when it should not? How/why does this happen?

For example, the same owner has had it for years, but if he transfers the domain, might some registrars make that date update? I know they normally do not, but I wondered if there are some quirks (like letting a domain go past expiration date) that cause a reset.

This used to happen. I'm not sure if it does any more. People were very hesitant to transfer if their creation date got messed with.

Some registrars (e.g. Network Solutions) will change the creation date in their "registrar" WHOIS. However, the WHOIS at the registry level (at www.internic.net) will still show the true creation date.
