Can anyone recommend a free IM that is lightweight, ad-free and allows me to restrict access by IP address?

Try Trillian. I am not sure about the IP block.

Trillian - I don't think the block IP but the programs it uses (AOLIM, ICQ, Yahoo IM etc..) may block IPs.
